The following was received by the Cheshire Public School District:
Cheshire students entering pre-k and kindergarten are required by state law to have a complete physical exam within one year prior to school entry. The Yellow Health Assessment form for Pre-K and the Blue Health Assessment form for Kindergarten must be submitted to the school nurse prior to entry. This exam must include the immunizations required by the state. For more information, please call your child’s school nurse.
All students entering grade seven for the 2012-2013 school year must have a complete physical exam, including the immunizations required by the state, which are as follows:
2.Second dose of measles, mumps, and rubella
3.Written documentation of Varicella (chicken pox) or Varivax vaccine, second dose.
4.Meningoccal vaccine – one dose
No students will be admitted to school in August without the above mentioned documentation. This exam must be dated after May 1, 2012, if possible. These Blue Health Assessment forms must be received by the school nurse no later than August 15. The school administrator is required to send any student home if such requirements are not met by the first day of school.
All students entering grade 10 for the 2012-2013 school year must have a completed Blue Health Assessment form dated May 1, 2012, or later, if possible. All students in grades seven and eight who are participating in athletics are required to have physicals. Physicals for eighth graders only will be given by the Cheshire Public Schools’ medical advisors, in the Dodd Middle School Nurses offices on the following dates:
August 15 — 1 p.m. for all girls sports; grade 8 only
August 16 — 1 p.m. for all boys sports; grade 8 only
Students in grade seven must have physical examinations from their private physician.
All students in high school who are participating in band or athletics are required to have physcials. These physicals for students in grades 9, 11, and 12 will be given by the Cheshire Public Schools’ medical advisors, in the nurse’s office of Cheshire High School. All grade 10 students must receive a physical examination from their private physicians.
The schedule for students in grades 9, 11, and 12 to receive their physicals is as follows:
August 8 — 9 a.m. for band
August 9 — 9 a.m. for football
August 15 — 9 a.m. for girls cross country, volleyball, swimming, soccer, field hockey, cheerleading, and Powder Puff
August 16 — 9 a.m. for boys soccer and cross country
Please be sure to pick up a receipt from Nurse Ruddy upon turning in a completed physical form. This receipt is then given to the band director or coach, informing them that the student’s physical is on file and up to date.
