Student Physicals Must Be Completed Before Opening Of School

Cheshire students entering pre-K and kindergarten are required by state law to have a complete physical exam within one year prior to school entry. The Yellow Health Assessment form for pre-K and the Blue Health Assessment form for kindergarten must be submitted to the school nurse prior to entry. This exam must include required state immunizations. For more complete information, please call your child’s school nurse.

Grade 7:
All students entering grade 7 in September must enter school with a complete physical exam including the required state immunizations
1.Tdap booster
2.Second dose of Measles, Mumps, Rubella
3.Written documentation of Varicella (chicken pox) or Varivax vaccine, 2nd dose.
4.Meningoccal vaccine – one dose
No students will be admitted to school in August without the above documentation. This exam must be dated after May 1, 2011, if possible. These Blue Health Assessment forms must be received by the school nurse no later than August 15. The Principal will send the student home if such requirements are not met by the first day of school.

Dodd Middle School sports physicals, grade 8 only:
All eighth grade students participating in athletics are required to have physicals. These physicals will be given at Dodd Middle School nurse’s office by Cheshire Public Schools and School Medical Advisors.
The schedule is as follows:
August 24 1 p.m. All girls sports, grade 8 only
August 25 1 p.m. All boys sports, grade 8 only
All seventh grade students must receive a physical examination from their private physician.

Grade 10:
All students entering tenth grade must have a completed Blue Health Assessment form dated May 1, 2011, or later, if possible. This form must be received by the school nurse no later than August 15. The Principal will send the student home if such requirements are not met by the first day of school.

Cheshire High School fall band and sports physicals, grades 9, 11, and 12:
All students, grades 9, 11, and 12, participating in band or athletics are required to have physicals. These physicals will be given at Cheshire High School nurse’s office by Cheshire Public Schools and School Medical Advisors.
The schedule is as follows:
August 11 9 a.m. Football
12 p.m. Band
August 24 9 a.m. Girls Cross Country
Girls Volley Ball
Girls Swimming
Girls Soccer
Girls Field Hockey
Girls Cheerleading & Powder Puffs
August 25 9 a.m. Boys Soccer
Boys Cross Country
Please be sure to pick-up a receipt from Nurse Ruddy upon turning in a completed physical form. This receipt then is given to the band director or coach informing them that the student’s physical is on file and up-to-date.
All tenth grade students must receive a physical examination from their private physician.
